Ads is not a "set and forget" system. If your business doesn't change much, then Google Ads might not need a lot of time once your account has been well optimized.
Bidding up relevant keywords
Something that happens naturally as competition increases. In certain industries and markets this is much more noticeable than others. There isn't much benefit to anyone to bid up a keyword beyond its actual value though--that just loses money.
Tracking Google Ads can be difficult
Because we've been programmed to believe we should be able to see a 1:1 correlation between a sale and activity in AdWords. Google has gone to great lengths to try to improve the overall tracking for AdWords, but there are many cases where the "billboard" effect cannot be directly tracked. If sales go up when you are using AdWords, but go down when you are not, there is probably more than coincidence at work.

If your business doesn't change much, then Google Ads might not need a lot of time once your account has been well optimized. However, you'll still need to spend at least an hour or two every month going through search query reports for keyword expansion and negative keyword purposes. I have accounts that I've managed for more than five years that still have some bogus search queries every month. They are certainly more nuanced now, but that just means it can even take a little more time to determine whether the query is a good one. Even with those long running accounts I still have to do a deep dive about once a quarter to make sure they are running at their peak performance.
